Request Records
To get a medical record at no cost, patients can access, download and print their medical record by logging onto their MyMountNittanyHealth patient portal. To request a MyMountNittanyHealth account, visit mymountnittanyhealth.com or contact our patient portal support services at 814.231.7103, or myMNH@mountnittany.org.
Mount Nittany Health has partnered with CIOX Health, the nation’s largest provider of release of medical information services, to process and fulfill your request for a copy of your medical record. Beginning May 1, 2021, patients requesting their records for personal use from our medical records office, through the following ways, will be charged a fee set at federal rates:
- By phone at 814.234.6167
- By email at medicalrecords@mountnittany.org
- By walk-in appointment: Monday - Friday, 8:00 am - 5:00 pm
The average turnaround time is 2-3 business days, but may be up to 7 business days due to higher request volumes.
The release form can be emailed to medicalrecords@mountnittany.org, faxed to 814.231.7098 or sent via US mail to the following address:

If you were born at Centre County Hospital, Centre Community Hospital or Mount Nittany Medical Center and would like to obtain a copy of your birth certificate, please contact:
