Please bring your insurance card, list of current medications, list of past surgeries or procedures, list of past admissions to hospitals and identification. Try to arrive 15 minutes early to complete a medical history form.
In addition, please arrange to have your medical records forwarded to the office you will be visiting from your current healthcare provider. Mail or bring to us any records you have at home before your first visit.

Patients may request a copy of their medical records by contacting the office where they are seen.
If we refer you to another physician, we will copy your records and forward them without charge. Please specify names and locations where records are to be sent; accurate information is essential.

Whenever possible, we ask that you request prescription renewals during an appointment. If you call to renew your prescription, please do so in advance so you don't run out of medication while your request is being processed.

Mount Nittany Physician Group has a billing department that handles the accounts of our patients. If you are treated by a Mount Nittany Physician Group physician while a patient at Mount Nittany Medical Center, you will still receive bills from both your physician and the Medical Center.
